Welcome to Martial Arts and "S.T.R.O.N.G Programs" (including "S.T.R.O.N.G. Girls; S.T.R.O.N.G. for Guys; S.T.R.O.N.G. Leadership, and Girl Grit") offered in your school! We look forward to working with your family!

* There are NO Fees associated with UGDSB Programs (unless otherwise indicated by your program instructor - eg. Continuing Ed - $20 operation fee)

* This form MUST be completed prior to students' participation in the program
